By using careguard.ai you agree to these terms. They are short because the site is informational.

What this site is

An information site published by CareGuard, a Patient Safety Organization listed by the Agency for Healthcare Research and Quality (PSO P0268), operated by Sevadar Foundation Inc., a 501(c)(3) nonprofit.

What this site is not

Not legal advice. The pages describing attorney work product and patient safety work product describe how a workflow is designed and what a statute says. They do not tell you how a court will rule on your document, and no attorney-client relationship is created by reading them or by using the reporting channel.
Not medical advice. The pages on deprescribing, restraints and monitoring describe a clinical discipline and CareGuard’s positions. Nobody should start, stop or change a medication based on them. That is a conversation with a prescriber.
Not a regulatory filing route. A report submitted through this site goes to a facility’s ownership and its attorneys. It is not a filing with any government agency and does not discharge any mandatory reporting duty. Official routes.
Not an emergency service. Nothing here is monitored in real time. Call 911.
Not a certification, accreditation or endorsement of any facility. CareGuard issues none of those. What CareGuard is not.

Using the reporting channel

Report in good faith. Knowingly submitting false information helps nobody and may expose you to liability CareGuard cannot shield you from.
Do not submit a resident’s identifying health information where the situation can be described without it, and do not submit photographs of residents.
CareGuard does not retain reports and therefore cannot confirm receipt, provide status, or resend.

Content and marks

The text, structure and images on this site belong to CareGuard and Sevadar Foundation Inc. unless attributed otherwise. The “Listed PSO” mark, where used, is AHRQ’s and is used only in relation to CareGuard itself, as AHRQ requires. Quotations from federal statutes, regulations and AHRQ publications are quoted as public government material and attributed where quoted.

This site links to government pages, to the AHRQ listing and to related organizations. CareGuard does not control those sites and is not responsible for their content.

Disclaimer and limits

The site is provided as is. To the extent the law allows, CareGuard and Sevadar Foundation Inc. disclaim liability for loss arising from use of the site or reliance on its content. Nothing here limits any right you have that cannot be limited by agreement.

Text messaging

CareGuard operates a text messaging program for its own patients and for people who have contacted us directly. By giving us your mobile number — on a form, at the front desk, or in conversation with our staff — you agree to receive text (SMS) messages from us at that number. Consent is not a condition of receiving care or of purchasing any service.

What we send. Appointment scheduling, confirmations, changes and reminders; notifications about your visit, including paperwork we still need; follow-up and care coordination after a visit; account and billing notifications; and replies to a message or request you or someone acting for you has made. We do not send marketing or promotional texts, and we do not send bulk or broadcast messages. We do not text people who have not given us their number, and we do not add numbers to any list.

Message frequency. Message frequency varies. It depends on your appointments and where you are in a course of treatment. We do not send a fixed number of messages per week or month.

Message and data rates. Standard message and data rates may apply, according to the plan you hold with your mobile carrier. Those charges are yours, not ours.

How to stop. Reply STOP to any message from us to unsubscribe. We will send one confirmation and then stop, except where a message is required by law or is a necessary administrative notice. Reply HELP for help, call (314) 250-5100, or contact us through the contact form. You can also ask any staff member to remove your number.

Carriers are not liable for delayed or undelivered messages. Delivery is not guaranteed.

Text messaging is not secure and is not monitored around the clock. Do not send medical or personal health information by text, and do not use text messaging for an emergency. If you are having a medical emergency, call 911 or go to your nearest emergency department. If you are in crisis or thinking of harming yourself, call or text 988, the Suicide and Crisis Lifeline.
