What this site is
An information site published by CareGuard, a Patient Safety Organization listed by the Agency for Healthcare Research and Quality (PSO P0268), operated by Sevadar Foundation Inc., a 501(c)(3) nonprofit.
What this site is not
- Not legal advice. The pages describing attorney work product and patient safety work product describe how a workflow is designed and what a statute says. They do not tell you how a court will rule on your document, and no attorney-client relationship is created by reading them or by using the reporting channel.
- Not medical advice. The pages on deprescribing, restraints and monitoring describe a clinical discipline and CareGuard’s positions. Nobody should start, stop or change a medication based on them. That is a conversation with a prescriber.
- Not a regulatory filing route. A report submitted through this site goes to a facility’s ownership and its attorneys. It is not a filing with any government agency and does not discharge any mandatory reporting duty. Official routes.
- Not an emergency service. Nothing here is monitored in real time. Call 911.
- Not a certification, accreditation or endorsement of any facility. CareGuard issues none of those. What CareGuard is not.
Using the reporting channel
- Report in good faith. Knowingly submitting false information helps nobody and may expose you to liability CareGuard cannot shield you from.
- Do not submit a resident’s identifying health information where the situation can be described without it, and do not submit photographs of residents.
- CareGuard does not retain reports and therefore cannot confirm receipt, provide status, or resend.
Content and marks
The text, structure and images on this site belong to CareGuard and Sevadar Foundation Inc. unless attributed otherwise. The “Listed PSO” mark, where used, is AHRQ’s and is used only in relation to CareGuard itself, as AHRQ requires. Quotations from federal statutes, regulations and AHRQ publications are quoted as public government material and attributed where quoted.

This site links to government pages, to the AHRQ listing and to related organizations. CareGuard does not control those sites and is not responsible for their content.
Disclaimer and limits
The site is provided as is. To the extent the law allows, CareGuard and Sevadar Foundation Inc. disclaim liability for loss arising from use of the site or reliance on its content. Nothing here limits any right you have that cannot be limited by agreement.
Governing law
These terms are governed by the laws of the State of Missouri.
