For suspected abuse, neglect or exploitation of an adult in Missouri, call the Missouri Adult Abuse and Neglect Hotline at 800-392-0210. For an independent advocate for a long-term care resident, contact the Missouri Long-Term Care Ombudsman Program at 800-309-3282.
What to expect
- If you are a facility owner or administrative group, the first conversation is about the building — licensure type, census, whether there is a memory-care unit, and who at the ownership group should receive findings. Request a safety review.
- If you want to report something about a facility, the contact form is the wrong tool. Use the reporting channel, which routes to that facility’s ownership and counsel rather than to us. Report a concern.
- If you have a question about the PSO listing, the public record is at AHRQ and the answer is probably on the P0268 page.
What CareGuard cannot help with by email
- It cannot tell you whether a particular facility is safe. It publishes no ratings and no list of facilities. What CareGuard is not.
- It cannot give legal advice about a report, a privilege question or a facility’s obligations.
- It cannot give medical advice about a resident’s medications or care.
- It cannot act on an incident report — it does not keep them. What CareGuard does not keep.
